Non-Call Months

During non-call months, residents generally have weekends off. They have occasional assignments of “back up” call during these times, so that if a colleague on a call service needs to be out for a family event, interview, illness or other reason, there is a system for coverage. This is monitored for proper use.

Each team does one week per month of nights (5:00 p.m. – 7:45 a.m.) with teaching rounds 6:45 – 7:45 a.m. Most other days are 6:00 a.m. – 6:00 p.m. When one member of the team is off, the other team members cover. New patients are accepted nearly every day
Conferences: Daily attending rounds, Academic half day (Tuesdays, 9:15 a.m. – 12:30 p.m.), morning report (Monday and Wednesday, 10:00 – 10:30 a.m., Thursday, 10:00 – 11:00 a.m.), grand rounds (Fridays at 8:00 a.m.) and selected conferences at noon.

Call q 5. On call, senior and one intern leave by 9:00 p.m. The other intern stays overnight (5:00 p.m. – 8:00 a.m.) staffing admissions with the NF resident and nocturnist. Admissions are daily, except for pre-call. The typical day runs from 6:00 a.m. to 5:00 p.m.
Conferences: Daily rounds, Academic half day (Tuesdays, 9:15 a.m. – 12:30 p.m.), morning report (Monday, Wednesday and Thursday, 9:30 – 10:00 a.m.), grand rounds (Fridays at 12:00 p.m.).
VA Nightfloat – PGY-1 Two Interns per Month

Two interns share all of the nights, typically with a four nights on, four nights off schedule. There are no clinics this month, but they do have assigned readings and attend Academic half day, if not post-call.
Conferences: No formal schedule, but nocturnist developing a curriuclum to discuss and present for supervision. Attend Academic half day (Tuesdays, 9:15 a.m. – 12:30 p.m., when not post-call or on-call.

VA Nightfloat – Senior  1-2 Seniors (PGY-3) 15-20 shifts per month, 6:00 p.m. – 8:00 a.m., supervising the admitting intern and nightfloat intern, along with doing some independent admissions and the nocturnist.
Conferences: Informal teaching with the nocturnist.

We were part of the iCOMPARE trial flex arm and have been doing Q4 overnight call since July 2015 for one intern/senior pair, generally 6:00 – 10:00 a.m. the following day with a two hour nap while on-call. On the other two non-long/post days, one team is "short" (they leave around 2:00 p.m.), and the other is  "medium" (they leave around 4:00 p.m.).
Conferences: Monthly – Sim center central line, codes, ventilator and renal placement therapy. Daily – rounds and daily discussion of assigned readings from the Fundamentals of Critical Care course book.

Q4 traditional 24+4 (based on resident preference) call with every post-post day off. Schedule reviewed annually and is one of the highest rated rotations.
Conferences: Daily rounds + didactics. No Academic half day, unless it falls on post-post day off.
