It is the policy of the University of Arizona College of Medicine – Phoenix that students must attest to the  following annually:

Students who do not complete the mandatory attestation within the designated time frame are referred to the Student Progress Committee as a breach of professional conduct (Student Progress Committee Procedures and Process for Dismissal Policy).
