Student Record Review Policy

It is the policy of the University of Arizona College of Medicine – Phoenix (COM-P) to allow students access to their own records and to request release of records in accordance with the University of Arizona policies and FERPA guidelines. 

Process

Students can view their academic record by logging into the 'Student Center' in UAccess.  Requests for records to be copied or released to a third party can be made by  submitting a completed Authorization for Release of Information form to the COM-P registrar.
