Our team partners closely with primary and specialty care teams to support patients and families through complex medical decision-making, symptom management and alignment of medical care with each patient’s values and goals.
We care for patients with a wide range of diagnoses and across diverse clinical settings, including oncology, advanced heart and lung disease, neurologic illness, liver disease. transplant evaluation, critical illness, trauma and complex chronic conditions.
Palliative care is delivered through a collaborative interdisciplinary model that includes physicians, nurse practitioners, physician associates, social workers and additional support services — such as spiritual care and music therapy. Together, we work to ensure that care plans reflect each patient’s goals, preferences and values.
