Where do I submit my application and any other required documents?

Once you have updated your FAFSA, please complete the application form (PDF). 

If needed, submit required documents to COMPHX-FinancialAid@arizona.edu with the subject HRSA Loan Documents.

If you have questions, please email the College of Medicine – Phoenix Financial Aid Office at COMPHX-FinancialAid@arizona.edu.

Submit parental demographic, household, income and asset information as required:

• Provide parental financial information on form provided.
• The student's parental information must be submitted upon submission, a copy of their parent(s) signed tax forms or the Parental Certification of Non-filer must be submitted to determine eligibility.
